1. Approx. 1gm of Gold sample with Your Lab code: is being sent to you for testing.
2. Initial preparatory procedures such as cutting, cleaning as per your usual practice may be followed prior to testing.
3. Only the Fineness of Gold to be reported in PPT in the result submission form.
4. Treat the sample as routine sample and adopt IS 1418: 2009 (Gold testing by Fire assay) for testing.
5. Your Lab code allotted has being stated in this form. For future purposes, kindly quote this code.
6. Last date of receipt of results from you is 25th February 2021. Result received after this date will not be considered for evaluation.
7. If you are not satisfied with your performance evaluation in this round you can appeal within 20 days of the report issue. Your appeal would be dealt with consideration.
8. Gold residues need not be returned back. Please find attached QR. PTRF.03 for your result submission

a) The details filled in this registration form by the participating laboratory will be treated as final for all the steps throughout this PT round.
b) The PT item would be dispatched to the address as stated in this registration form.
c) Being high value metal-the PT item (Gold) would be sent through insured post. Kindly acknowledge the receipt of the same.
d) Participants are required to test the PT item only by the method specified. Results reported by method(s) other than specified would be treated as invalid and no refund of the fees would be made.
e) Non submission of test reports by the stipulated date would lead to dis qualification of the laboratory and no refund of the fees would be made.
f) Either hard copy legibly filled or filled in-soft copy scanned has to be sent to confirm the registration.
g) Authenticate this form with signature, date and seal of your firm.
